There are a few forms needed before your child can participate in any football or cheer activities after July 15th.

• All new players must provide a copy of their original birth certificate. This must be the official government document, and not a ceremonial one provided by the hospital. Returning Argos players do not need to provide this.  
• A sports physical completed within the previous 12 months. There is no official Pop Warner Physical Form this year, any sports physical form provided by your child's medical provider will be sufficient. We have a link to the current IHSA Sports Physical Form below for your convenience. 
• Your child's final report card of the 2025-2026 school year, showing ALL 4 QUARTERS of grades. 
• All Coaches need to complete the Volunteer Form (link below) and submit by July 14th, 2026. 

The sooner you can submit these forms, the better, but all forms must be completed by July 10th, 2025 for both football and cheer. If forms are not completed your athlete will not be allowed to participate.
